Design and implementation of a chart review for the clinical care process in radiotherapy

Abstract
Radiotherapy is a clinical care process that integrates clinical, technical, and dosimetric stages; therefore, it requires review mechanisms to ensure the consistency of information throughout treatment. In this work, a chart review was designed and implemented for the radiotherapy service of the S.E.S Hospital Universitario de Caldas, with the aim of organizing the review of the care process, supporting the identification of inconsistencies, and strengthening patient safety.
The tool was developed in Microsoft Excel based on the recommendations of the American Association of Physicists in Medicine (AAPM) TG-100 report and the TG-275 report These reports were reviewed, filtered, and adapted to the actual workflow of the service.
For its development, the verification items were organized according to treatment modality, review stage, and professional role, considering external beam radiotherapy and high-dose-rate brachytherapy. As a result, an interactive interface was obtained, integrating checklists, compliance criteria, progress percentages, observation fields, and a monthly patient registration and control module.
This tool makes it possible to centralize the review of the radiotherapy process and facilitate monitoring before, during, and after treatment. The developed chart review provides an operational basis for improving traceability, guiding structured review, and progressively contributing to a culture of patient safety within the service.
